Neuroendoscopy-A Minimally Invasive Way of Brain Tumor Surgery

Neuroendoscopy for brain tumor

What is Neuroendoscopy?

Neuroendoscopy is one of the most advanced minimally invasive brain surgery methods available today. This is a least invasive brain surgery method which is mostly used to successfully treat brain tumors in a person.
This minimally invasive surgical technique involves drilling a small (10-12 mm) hole in the skull through the nose (or mouth) instead of removing a large part of the skull to perform the required tumor removal and treatment for brain tumor.
This surgery is performed using a special surgical device known as an ‘Endoscope’. This consist of a small and flexible surgical tube which is attached with a small but high-definition video camera at one end and a viewing-port at the other end. This allows the surgeon to control the movement and direction of the endoscope and guide it to the correct spot inside the skull.

What is Neuroendoscopy useful for?

Neuroendoscopy is most useful in initially performing a biopsy on a suspected brain tumor using minimally invasive methods. The biopsy confirms or denies the existence of the brain tumor using detailed pathological study and analysis at the laboratory.
This least-invasive surgical method is also useful in treating various kinds of brain tumors, especially those that are seen growing in hard-to-reach places inside the skull or where there is high risk of brain damage due to the close proximity of the nerves in the brain.

What are the types of Brain Tumors treated with Neuroendoscopy?

These are the various types of brain tumors that are successfully treated using minimally invasive Neuroendoscopy methods:
  • Ventricular tumors
  • Skull-base tumors
  • Rathke’s cleft cysts
  • Pituitary gland tumors
  • Pineal region tumors
  • Metastatic brain tumors
  • Acoustic neuromas

What are the advantages and benefits of Neuroendoscopy?

Neuroendoscopy is a comparatively safer and advantageous brain tumor surgical treatment than the traditional brain tumor removal methods. At times, certain brain tumors develop in close proximity to vital neurovascular systems (nerves) which increases the risk of damage and the resultant grave complications arising out of it. Although, the recent microscopic brain surgery method is mostly useful it still faces difficulties in treating brain tumors that are located in hard-to-reach places inside the skull as it provides live viewing of the surgical site.
The wider and clearer view provided by an endoscope as compared to a microscope is more advantageous in viewing the operational site in detail and avoid damaging the surrounding nerves or other brain structures.
These are the distinct benefits of a minimally invasive Neuroendoscopy surgery for brain tumor:
  • Less painful
  • Less scarring
  • Quicker recovery
  • Shorter hospital stay
  • More accessible for surgery inside the skull
  • Helpful in removing tumor without damaging surrounding healthy vital brain structures
  • Improved viewing of operational site inside the cavities in the brain structure
  • Better special surgical tools ensure decreased risk of complications from surgery

Different Available Options for Breast Lift Surgery in India

breast-lift-surgery

Breast Lift Surgery

A breast lift surgery is used to address uneven or sagging breasts with reduced volume and stretched areolas to give the breasts a different and more youthful appearance.
In case the cosmetic surgeon feels that your breasts do not have sufficient volume they may also suggest a breast augmentation or a breast reduction in case there is excess fat deposits in the breasts. This is one of the most popular cosmetic surgeries in the world and thousands of women across the world undergo this procedure every year.

How Breast Lift surgery works?

A breast lift surgery is performed usually after childbirth or a drastic weight loss in order to reshape the breast to be proportionate to the new and slimmer body shape.
A breast lift surgery works by two basic methods. The first instance is where the breast volume is drastically reduced and requires a fat transfer or an implant to increase the size and reshape the breasts as desired by the patient. The second instance is when there is excessive fat deposit in the breasts (usually after a childbirth or a drastic loss in weight) the surgeon will require to remove the excessive fat deposits as well as tighten the loose skin to reshape the breasts while reattaching the nipples and areolas according to the newly acquired shape and size of the breasts.

Breast Lift Surgery procedures

The breast lift surgery is performed under general anesthesia to prevent discomfort to the patient. The cosmetic surgeon will then mark the pre-decided locations where incisions are needed. These are small incisions that are used to insert specialized surgical instruments. These tiny surgical instruments are used in a minimally invasive surgical procedure for a breast lift surgery.
The surgeon will use these instruments to remove the extra skin from around the breast and stretch your breast tissue into the new position. In case of breast augmentation with an implant the cosmetic surgeon will insert a breast implant made from silicone filled with sterile saline solution through these incisions and under (or over) your breast muscles.
After the surgeon is satisfied that the breast size and shape is achieved as decided they will sometimes need to relocate the nipples and areolas in accordance to the new size and shape of the breasts. The surgeon will then close these incisions with a bandage and attach a drainage tube to the incision site in order to allow any excessive accumulation of fluid to drain out from the chest area.
A day or two later the surgeon will remove the bandages from the incisions and check the nipples for adequate blood supply. A few days after the surgery tends to make the breast area feel uncomfortable and numb and the doctor will usually advise you to wear a special dressing to provide support to the breasts during the recovery period.

Who is a candidate for Breast Lift surgery?

A breast lift surgery is considered ideal for you if you are unsatisfied with the shape and size of your breasts due to their becoming less taut and saggy. The surgeon may suggest a combined breast augmentation and breast reduction procedure for reshaping the contours of the breasts to desirable one.
These are the most common indications as to why you should consider a breast lift surgery:
  • Your breasts seem inadequate in size and become pendulous.
  • Your breasts have lost firmness and are too saggy.
  • Your nipples and areolas point downwards and are located below the crease of the breast.
  • Your breasts are uneven in size and shape.
  • Your breasts differ in size from each other.
  • Your breasts seem abnormally small.
  • Your breasts are too large and need to be reduced to more normal size.
  • You have no further plans to get pregnant.
The surgeon and cosmetic specialist will consider your medical history and overall health at the time to decide of you are ready to undergo a surgery.

Benefits of Breast Lift surgery in India

  • A breast lift surgery is mostly an aesthetic procedure that involves reshaping and decreasing/increasing your breast size to suit your expectations so that you look and feel more youthful.
  • There are various benefits of a breast lift surgery such as the firm feeling in your previously sagging breasts will certainly boost your confidence and self-esteem.
  • A breast augmentation helps you get proportionate-looking breasts in case your breasts were too small or too big in size.
  • The breast lift surgery helps improve your outward appearance and gives you a more youthful appearance.
  • A breast lift surgery is performed with excellent results in the vast networks of cosmetic specialty hospitals and clinics that are situated in the major cities of India such as Delhi, Mumbai, Chennai and Kolkata among others.

Complications with Breast Lift surgery

A breast lift surgery is similar to any major surgical procedure and also faces the risk of certain complications that might arise out of the surgery.
These are the few uncommon risks and complications that are related to a breast lift surgery:
  • Anesthetic reaction
  • Infection at incision sites
  • Internal bleeding (hemorrhage)
  • Fluid accumulation under the skin (seroma or hematoma)
  • Scarring
  • Tissue or nerve damage
  • Blood clot
  • Unsatisfactory results of the surgery
The cosmetic surgeon and your consulting doctor will take every possible precaution to avoid giving rise to these complications.

Proximal Hip Replacement Surgery – What Result Can I Expect?

Proximal hip replacement

Proximal Hip Replacement

A proximal hip replacement surgery, also known as hip arthroplasty, is a surgical procedure that aims to restore strength and function back in a disabled, or damaged, hip joint. This is a better option for people affected with bone degenerative disease such as arthritis who experience pain and disability due to a damaged hip joint.
This surgery aims to replace the worn out, or damaged part, of the hip joint with an artificial prosthetic that functions just like a natural hip joint. This surgery is helpful in relieving pain caused by a damaged hip as well as in restoring strength and range of motion in the joint.

Hip Joint

A hip joint is a normal ‘ball and socket’ joint but is one of the most important and weight-bearing joints in the body. It is one of the largest joints in the body and is formed by the acetabulum (part of pelvic bone) which makes the socket part while the ball part consists of the round head of the femur (thigh bone) on the upper end.
These two bones surfaces are covered with soft articular cartilage, called a synovial membrane, which is composed of a smooth and soft tissue that acts as a cushion between the ends of the bones and enables lubrication for free movement of the hip joint.
The hip joint is stabilized with a network of strong tissues called as ‘ligaments’ connecting the ball to the socket.

Causes of Hip Pain

The most commonly seen cause for constant (chronic) hip pain and dysfunction is arthritis, which develops into several different types, such as:
  • Osteoarthritis – This type of arthritis is related to the wearing out of the bones and the cartilage due to aging. It is normally seen in people above the age of 50 years as well as in younger people with a family history of arthritis. This type of arthritis mainly causes the soft cushioning cartilage to wear away and this causes the joint bones to rub directly against each other. The rubbing of bones in such a manner results in stiffness and pain in the hip joint.
  • Rheumatoid arthritis – This auto-immune disease causes the synovial membrane (soft cushioning tissue) to become inflamed and swell. This chronic swelling results in severe damage to the cartilage and causes stiffness and pain in the hip joint. This type of arthritis belongs to the group of ‘inflammatory arthritis’ diseases.
  • Post-traumatic arthritis – This type of arthritis is commonly seen accompanying a serious trauma or injury to the hip region resulting in fracture of the hip joint bones. The cartilage might also be damaged and can cause pain and stiffness in the hip joint.
  • Avascular necrosis – This type of arthritis develops due to lack of blood supply to the femoral head of the thigh bone and is also called as ‘osteonecrosis’. This is usually the result of a severe injury to the hip or a fracture that may disrupt the blood supply to the femoral head. This causes the femoral head’s bone surface to collapse and result in arthritis.
  • Congenital hip disorders – Some infants or children may have hip problems right since their time of birth and may have been treated but are yet capable of causing problems in the musculoskeletal structure of the body. This is often the result of the joint surfaces being affected during development.

Candidate for Proximal Hip Replacement surgery

Although there are no obvious restrictions on weight and age for a proximal hip replacement the surgeon/doctor will decide on this procedure based on certain factors, such as:
  • Disabling hip pain disrupts normal everyday activities
  • Constant hip pain during day or night
  • Hip become stiff and the range of movement gets restricted
  • Unsuccessful previous treatment methods such as anti-inflammatory drugs, painkillers, walking devices and physical therapy
If you are being considered for a proximal hip replacement surgery, the surgeon/doctor will perform certain diagnostic tests to determine the extent of damage and the repair needed for it. Your medical history will be evaluated to gather information pertaining to your general health and other prevalent medical conditions, if any.
A physical examination in followed by x-ray tests to identify the location and extent of the deformity in the hip joint.

Proximal Hip Replacement procedure

This is a major type of orthopedic surgery using artificial implanting techniques for treating the painful and disabling symptoms of damage caused to the bone of the hip joint.
The surgeon will use an artificial hip joint prosthetic to replace the damaged natural hip joint components such as the femoral head and the acetabulum. This implant is made from a wide variety of materials and comes in different shapes and designs. The implant component may be made from ceramic, medical-grade metal, plastic or a combination of these. Some implants need to be attached with a stabilizing pin to the femur and the acetabulum while other devices can be attached using surgical adhesive. Any type of an artificial hip joint prosthetic will contain two main parts: the ball component, to be fitted on the femoral head, is made of ceramic material or of a strong and highly-polished metal and the socket component which is made from durable ceramic, metal or plastic with an outer covering made from strong metal.
The proximal hip replacement procedure begins with the administering of general anesthesia so as to avoid any discomfort or pain to the patient during the surgery. After this the surgeon will make a few small incisions along the damaged hip joint. These incisions will be used to gain access to the damages hip joint component bones after shifting aside the blood vessels, nerves and other soft tissues.
The surgeon will remove any cartilage debris and remaining bone pieces from the hip joint and shave, or scrape, the surface of the femoral head to reshape it in order to fit the ball component of the artificial hip joint prosthetic. The next step involves reshaping the acetabulum (socket) part of the pelvic bone to help fix the socket part of the artificial prosthesis. After the surgeon ensures that these components are firmly fixed to their respective joint bones the artificial femoral head is inserted into the prosthetic acetabulum socket.
The surgeon will then check to see if the movement of the hip joint is restored and close the incisions using fine sutures. It will take 2-4 weeks for the implants to get completely covered by the bone.
A proximal hip replacement surgery is one of the more preferred forms of treatment for chronic hip pain and disability using artificial prosthetic components to replace the original hip joint. This surgery has been found satisfactory in relieving the severe pain and disability that is often caused by a severe case of hip joint disorder or injury.

Advantages & Disadvantages of Bilateral Knee Replacement Surgery

Bilateral knee replacement surgery is a procedure where both knees are replaced one after the other in a single operation session. However, it is quite uncommon for people to have arthritis in both knees, leave alone reaching the same degree at the same time & where bilateral knee replacement surgery may eventually be required. But even when such a case arises, the decision to undergo bilateral knee replacement is one of convenience since having a single newly operated knee along with one which is still stiff & painful from arthritis can make recovery more difficult.

Why Undergo Bilateral Knee Replacement Surgery?

Bilateral Knee Replacement Surgery
When cartilage has worn in both knees, an artificial knee joint known as prosthesis can take its place. Surgical intervention which is meant to implant prostheses in both knees is called Bilateral Total Knee Replacement Surgery. During this procedure, only the surface of joint are removed & arthritic ends of bones are shaved & replaced with newer plastic & metal surfaces. Bilateral knee replacement surgery is able to recreate almost normal function of knees, while the goal remains to relieve pain. This procedure is also meant to help restore motion of both knee joints while straightening limbs.
  • Simultaneous Bilateral Total Knee Replacement
Simultaneous bilateral total knee replacement procedure is designed to replace both knee joints of a patient on the same day, under single anesthesia. This usually takes place in the same surgical event within a single hospital stay & is normally followed by a common rehabilitation program.
  • Staged Bilateral Total Knee Replacement
Staged bilateral total knee replacement surgery means that although both knee joints are replaced, they take place on two separate surgical events. Moreover, each surgery is performed several months apart from one another & therefore requires two hospital stays, two anesthesia administrations & two separate rehabilitation programs.

Health Considerations While Deciding to Undergo Bilateral Knee Replacement

Patients should consider both pros & cons of bilateral knee replacement surgery before consenting to the operation. Bilateral knee replacement is a much longer surgery which takes about 3 – 4 hours of time to complete operating upon both knees. This is quite a long time upon the operating table & involves long anesthetic influence which may not be well tolerated by physically compromised patients like those who are frail & 80+ & others with pulmonary disease or cardiovascular problems.
  • Disadvantages of Simultaneous Bilateral Total Knee Replacement
Disadvantages of simultaneous bilateral total knee replacement procedure include a probable increase in risk of cardiovascular complications & a high possibility of requiring blood transfusion following surgery. Moreover, this procedure is not available to all patients with problems in both knees simply because it implies higher stress upon the cardiovascular system. Only those patients who are in good health are appropriate candidates & therefore special emphasis must be made for adequate patient selection. Moreover, initial rehab is slightly more difficult as patients are left with no good leg to stand upon.
  • Disadvantages of Staged Bilateral Total Knee Replacement
Disadvantages of undergoing staged bilateral total knee replacement surgery includes that the patient will require two different stays at the hospital, along with two anesthesia administration & delay in time for returning from disability.

Recovery Consideration Following Bilateral Knee Replacement Surgery

Early rehabilitation following bilateral knee replacement surgery during recovery can most often be hampered for patients as they would be having two legs which are difficult to move, bend or lift them. However, most patients will be able to adequately bear weight upon them. Results in terms of function & relieving pain in both types of bilateral knee replacement surgery procedures are excellent & equivalent for relieving pain & performing everyday activities. Most bilateral knee replacement patients can expect more than 95 percent chance of success at 15 years.

Benefits of Undergoing Bilateral Knee Replacement Surgery

Bilateral knee replacement advantages and disadvantages
  • Advantages of Simultaneous Bilateral Total Knee Replacement
Advantages of undergoing simultaneous bilateral knee replacement procedure include the fact that it involves a single surgical event alongside single administration of anesthesia, shorter stay at the hospital & possibility of symmetrically rehabilitating the patient.
  • Advantages of Staged Bilateral Total Knee Replacement
Advantages of undergoing staged bilateral knee replacement surgery apparently include lower level of stress for the cardiovascular system along with lesser complications relating to heart & deep veins & lesser possibility of requiring blood transfusions as well following surgery. Moreover, staged bilateral total knee replacement surgery is an excellent choice for patients with pulmonary, vascular or cardiac diseases or for those who are above 80 years of age.

Support Considerations Following Bilateral Knee Replacement Surgery

Bilateral knee replacement surgery requires a strong home team for patients, which is even more than a single knee replacement procedure. This can include a hired nurse & therapeutic support along with members of the household. Moreover, physical & emotional support for the patient is required for several weeks of time following bilateral knee replacement surgery & which is extremely vital for achieving proper recovery.

Affordable Bilateral Knee Replacement Surgery in India

Cost of bilateral knee replacement surgery in India is just a fraction of what an international patient would pay to undergo in developed countries like Australia, Germany, UK, Canada or the United States. Moreover, knee replacement surgery in India is of excellent quality with success rates at par with the best in the world. What you should know about Discectomy

A discectomy is a surgical treatment procedure that is useful in mainly relieving the painful and disabling symptoms that are caused as a result of a pinched nerve in the spinal column due to a herniated (damaged/bulging) inter-vertebral disc.

This surgery aims to remove the damaged portion of the disc from the spinal column. A herniated (bulging) inter-vertebral disc can lead to pain, weakness and numbness, especially in the legs initially, and can spread to the entire body if not treated efficiently.

This is a last-resort treatment in most cases when other alternate treatment methods have failed to relieve the symptoms being caused by the herniated inter-vertebral disc.

Need for Discectomy

The main aim of a discectomy procedure is to provide relief from the intense and constant pain and other disabling symptoms that accompany a herniated (bulging) inter-vertebral disc form the spinal column.

A discectomy will usually be advised if:

  • Patient has trouble standing/walking due to nerve damage
  • Other alternate treatment methods (medication, physiotherapy, etc) have been unsuccessful
  • A disc fragment (bone spur) gets lodged in the spine, causing nerve impingement (pinching)
  • Pain can be felt growing out from the buttocks to the arms and legs or even the chest

Risks associated with Discectomy

Although a discectomy is considered to be an ideally safe procedure, there are rare chances of certain complications that may arise due to the surgical treatment, such as:

  • Infection
  • Bleeding
  • Spinal fluid leakage
  • Nerve damage
  • Vascular (blood vessel) damage

Discectomy procedure

A discectomy will mostly be performed as a major surgical procedure requiring administering general anesthesia to prevent pain and discomfort to the patient during the surgery.

The patient is usually required to lie down on the stomach so the spinal column is easier to access. The surgeon will make incisions as per the requirement of the particular surgery. These incisions are used to gain access to the herniated (damage/bulging) inter-vertebral disc after carefully shifting aside the various back muscles and other soft tissue. The surgeon might also require removing small portions of the spinal column, or certain ligaments, to get access to the damaged disc.

Mostly, the surgeon will only remove the particular piece of inter-vertebral bone that is causing the impingement (pinching) on the spinal nerves, this relieving the constant pain and disability being caused as a result of this pinching of the nerves.

In case the surgeon requires removing the entire inter-vertebral disc, due to extensive damage to it, the surgeon will use a ‘spacer’ (made from grafted bone material/synthetic material) to fill in the gap left by the removed inter-vertebral disc. The surgeon will then ensure that the adjacent inter-vertebral disc can fuse together to restrict its motion, this helps to heal the inter-vertebral bone faster and prompt quicker recovery from the surgery.

This procedure is mostly performed as an outpatient procedure where the patient is allowed to go home the same day after the discectomy surgery in case the surgeon/doctor cannot identify any potential risks of complications.

The doctor will advise the patient to avoid lifting heavy weights and other strenuous exercises for a few weeks after the surgery.

The doctor might also advise a course of physical therapy in order to regain the lost strength and movement in the spine.
